To be an active member of the Drexel University Women’s Rugby team, each member must complete and submit the following forms to the Secretary prior to the start of the season.
Form 1: Each player must be CIPPed by USA Rugby.
Form 2: Each player must fill out and sign the Assumption of Risk and Release of Liability Form.
Form 3: Each player must fill out their contact information.
Form 4: If any player will be driving at any point during the season, they must submit this form.
Form 5: Each player must have a physical completed by their physician for the 2010-11 school year.
